PIO card for minor - US citizen entry into India

The PIO card process for minor requires parents to send a parental authorization form.

There are 2 dates that are required on this form which are the from and to dates within which the minor can travel from the US. What should these 2 dates be?
Can anyone help? We are sending the application to Washington jurisdiction.
