Visitor Visa Extension ... Pls help

I am trying to extend my parents visitor visa while they are in USA. Can you pls help on the following:

1. Do we need to notarize I-134 form ?
2. Where should we mail all the documents .. meaning which service center ? We stay in Florida.
3. I am not enclosing the return air tickets .. I am saying, once I have confirmation, I will book return tickets. I hope this is fine.

Please let me know. Thanks in advance.
