Visitor Visa extension - supporting documentation

Hi ,
I am applying for visitor visa extension for my mother-in-law. I have read through I539 instructions and gathered the required documenation. I have doubt on Form-I134 and return airline tickets.

Do I need to enclose I134 along with I539 for visa extension.
Reg. return tickets, we have return ticket for date prior to I94 expiry. Do I need to change this ticket to future date(i.e. as per the requested extension date plan) with airlines and enclose it? (OR) is it OK to enclose the ticket as per the current travel plan.
