Ideal time to apply for visitor VISA extension

Hi,

I am planning to apply for a VISITOR visa extension for my mother in law. Since, its just been a month since she arrived to USA. When should I apply for extension ?

(Keeping in to account that processing takes around 3-4 months), Should we apply after 2 months ? (Will this not be considered as pre-planned ?)

I think timing is fine, but the bigger question is, what is the reason for extension?

On an average, how many months will it take for visitor visa extension to be completed?

Also, if the visitor stays beyond the i94 date while the case is pending, is it legal?

If the extension gets rejected, what are the implications?
