How do I apply for B2 Visa Extension

I have a friend came to US with B2 visa. I-94 expires in 3 months. He wants to extend the stay here in US for additional 3 to 4 months.

1. Is it possible to extend B2 visa?
2. When should he send the application requesting extension?
3. What other supporting documents he should send with the application?
