Visitor visa extension during the first trip to usa

Hello,

I would like to know if anybody has extended the visitors visa during their first time travel to usa? Any rejections?
My mom has 10 yr visa and she is here now. I am planning to apply for extension. She got lot of rejections(214b) before getting a 10 year visa in 2004 march.
Do you think this might be a problem and they may not give her extension?.

PLease let me know your experiences and suggestions.
