Extend Visitor Visa I-134

Hi Admin,

In our 'Extend Visitor Visa' section there is a link to I-134 document. This document does not have a column for notary. I remember that I notarized the document last time when I my mom went to stamping. Can anybody give me the exact I-134 link.
