extend visitors' visa form

Hi,

I wanted to extend my Patents visitor's visa.
I Looked into Form 539 from this website but it looks like
it expires 7/31/2004. where can i get the new forms.
Please advise

Can I send Itinerary !!!!

What are the documents , Do I need to send with the application. Do I need to send the ticket,passport ,I 94.
or just Itinerary is enough for the proof.
Can I send Itinerary instead of return ticket.
