Visitor Visa Extension- Urgent Question

My parents are here on 3rd visit and this time we are planning to apply for an extension.

1. They have a 1 year open ticket, is it a good idea to mention that.
2. Can I get a sample reason letter. Whom to address and some kind of format.
3. Are we sure that if the visa extension is rejected the 10 yr visa is going to void. Do we know any link etc because I don't see any mention in the INS site.

Where we need to post the vistor visa extension paper if we live in Maryland. Also do we need to provide Affidavit of support ( I-131)
