Visitor visa extension I-539 questions

Hi Experts,

I am filing I-539 for visitor visa extension. I have following questions

1) I e-filed application for my mom. Should I send the signed copy of application again? or just the confirmation receipt?

2) Do I need to include I-134 with this form or just the pay stubs and bank statements suffice?

Just confirmation. Do not send another copy, they may consider it as separate application.
