Visitors visa /B2 -extension of stay /I 134 reg

1.I am applying for extension of stay using I 539. In item 11 of I-134 ,whether my daughter who is in US should tick"intend"?
If intend whether "board and room" or "money" is to be selected .If "money" how much dollars should be mentioned.
Whether salary certificate,bank statement ,previous year's IT return is to be submitted.Bank statement for how many months?

2.I had visited US 4 times during the last 3 years.Will extension be granted?

3.Should I enclose copy of existing return ticket ,date of return flight is november 22nd (MY I 94 date is Nov28th)
4. I am going to send request stating that I want to help my daughter who is undergoing training in medicine.Will the USCIS
enquire in the University ?

3. No, you need to send the copy of the new return ticket

4. That is not a good reason. Tourist visa is for tourism, and extending tourist visa is for more tourism.
