Visitor Travel and Medical insurance for me and my Wife in USA

I will be travelling to USA along with my Wife and I need a travel and medical insurance policy from a company in USA.

Please suggest me a policy to buy which has a good PPO coverage, better claims processing and acute on-set coverage (though the premium is on the higher side).

I am looking for 35 to 40 days time period.
