Help me understand - acute onset of pre-existing conditions

Hello,

I have parents visiting from India
Father age - 66
Mother age - 65

Both have pre-existing condition - diabetic, blood pressure

As I understand there is no plan which will cover pre-existing conditions.
But some plans cover acute onset of pre-existing conditions.
I want to know
1. How insurance company decides if it's acute onset or not?
2. in case of Acute-onset, will insurance cover required tests?
2. After paying deductible, does it works like normal insurance in US where hospital bills directly to insurance company if in network?
3. If i get into problem with claims, where I need to contact? Insubuy?

Why I can not buy coverage of 75000 to 85000 for my parents of age 66?

Could you please tell me exactly all the input criteria you entered so that I can find it out for you?

Most likely reason is that you want comprehensive coverage plan and no plan exists in that policy maximum range.

Coverage area - USA and during Travel
Policy maximum - 70000 to 85000
State - Texas
Start Date - 11/15/2016
End date - 01/23/2017
Age for both - 65 to 69
Arrival in US - yet to arrive

I entered the same criteria and I found 4 products, all fixed coverage plans.

Did you initially see any of those plans and then clicked on 'narrow search' and selected 'Comprehensive', by any chance?

Yes I did filter on comprehensive plan

Please try '$100,000 to $125,000' policy maximum.

I checked that and seems like restriction, either can buy below 70000 or above 100000 - 125000 but no option to select 70000 to 85000
