Yes, it is indeed necessary to get health insurance for visitors to United States.
Insurance is only for healthy people. If the person is already sick or have pre-existing conditions, insurance is not going to pay for it any way. Insurance company pays for any new medical conditions, sicknesses, injuries or accidents that may occur after the effective date of the policy. And the possibility of someone getting into completely new condition is same for every one for a given age.
However, when you get into new medical condition, the medidcal expenses are quite high in the United States. You would be taking yourself a big risk by not buying the visitors insurance.
You buy house insurance when nothing is wrong with the house. Just in case, there is fire, you would be covered.
You buy car insurance when your car is fine. Just in case, if your car gets damaged in an accident, you would be covered.
Same thing applies for visitors medical insurance as well.
