Medical Insurance for mother

My mother is 64 Years and will be travelling to US on 17th July . It would be great if you provide me more details for Protection America plan.

1. Do I need any Dr's check up from here (Indis).

2. Medical Insurance will be only for California ( Sanjose) or I can get her checked up any where in US incase required like I have PPO.

3. 250 deductible means when hospitalised or for any visit to Doctors.

4. Will I be able to insure her before we reach to US.
