Visitors Insurance and UK NHS

My dad is 79 years old and had a heart attack 6 years ago. He is in good health and has had no further problems although on medication. I know that insurance companies will not insure him but I have 2 questions. 1. will any company offer insurance for higher premiums if a doctor certified his current health as being good/stable? 2. He is a citizen of the UK - will the health program in the UK cover any incurred costs for medical treatment in USA (national health program in UK) thank you.
