Visitors medical insurance for mother

My mother is 60 yr old and will be visiting US and Canada planning to come here during Nov end. If she ends up going to both US and Canada her total stay may extend over 10 –12 months both places inclusive.

Can you please recommend what would be the suitable policy for her. Are there medical check ups required before buying the policy? What is covered?
