Clarification for visitors insurance required

I am starting from my house in Bangalore, India, around 21.00 hrs. on 24th of July to catch a flight to USA which is to takeoff around 02.00 on 25th July,
and I will be leaving my house in USA in the afternoon of 3rd Dec to catch a flight to Bangalore which will reach there around 01.30 hrs on 5th Dec.and I will reach home by about 03.30hrs.
Please let me know what dates I should fill up in your online booking for creating a quote.
Also please clarify whether the coverage can be from
21.00hrs of 24th Jul to 03.30 hrs of 5th Dec.
or is it restricted to only from takeoff at Bangalore(02.00hrs.on 25th Jul) to landing at Bangalore(01.30 hrs.on 5th Dec).
