Insurance for entire family - not married

I'm purchasing an Atlas insurance for me, the mother of my children and our 2 children (1 and 2 years). shall I fill the form adding her as spouse even if we are not married? Also, we are coming to the USA next week, do you send the policy and the contract via e-mail to have the insurance number? Thank you in advance,

You can't put yourself and mother of your children in the same policy as you are not married.

I suggest that you make a separate applications for you and her. And you add one child in each application.
App 1 = You + First Child
App 2 = Her + Second Child
