Need help filling a claim form

Hi I am trying to fill in the claim form but got stuck with the last page which says Privacy and confidentiality release form it says

I authorize IMG to discuss my claim activity with _________________(who's name should we give here - doctor? or some one from my family?)

That form does not need to be included when you submit your claim.

However, most likely, you have purchased the insurance for your relative and are going to deal with IMG and not really your relative such as parent. IMG is not allowed to discuss the claims matter with you until the insured authorizes you to do so.

Therefore, you can write your name in with _______________ and ask the insured to sign on it.
