No last name in the passport

Hi Narendra,

My mom will be reaching US on Jul 7th and i am going to go with Patriot insurance.

In her passport,

Surname: <Blank>
Given Name: Asha Rani

Since US Visa cannot have a Blank Surname, they used Given name in Surname and stamped US visitor visa as follows wher FNU denotes First name unavailable,

Surname: Asha Rani
Given Name: FNU

This is a common problem exist in most of teh south Indians who has no surnames, i am also sure you would have seen hundreds of your clients with the same issue. Now while applying for Patriot insurance i am planning to furnish the name as per passport (ofcourse i will not put Furst name as FNU though as it just denotes First name is unavailable),

So it is going to me,

Surname: Asha Rani
Given Name: <Blank>

Please confirm if i am right so that i can go ahead buying insurance for my mom and for my mother in law (mother in law dont have this issue).

You can put the name exactly as it is on the visa.

Or you can put
First Name: Asha
Last Name: Rani
In this case, it will print "Asha Rani" on the insurance card.

You can't leave any of the first name or the last name blank, as both are required.
