How does insurance company figure out pre-existing conditions?

Hello, I am looking for an insurance for a traveller of age 80. In that regards I would like to know that how does insurance company define if the traveller got sick after cominng to the US, does they require Doctor Certificate from the native country to make sure the traveller was 100% healthy when he/she arrived in the US.
