How are pre-existing conditions determined. My mother is coming to the US and has no health problems as far as we know. If she falls ill and needs to be taken to the hospital who would determine if it was a pre existing condition?

If your mother gets sick, when she goes to the doctor, depending upon the examination he does, tests he asks you to get done, whatever treatement he does, he sends attending physician report and send along with all the medical records.
Insurance company's claims department look at them and determines whether it was pre-existing condition or not. In other words, they go by doctor's notes and reccommendations.
If things get quite complicated, most insurance companies' claims departments have medical doctors on staff who help in determining it, communicating with your doctor in medical terminlogy etc.
