Medical Insurance valid in transit stop-over?

If a visitor's medical insurance is purchased from US, is that insurance applicable also if the traveller makes a 2-3 days transit stop-over enroute to US? (e.g. at London)

If you select 'USA', it will show all the plans that are valid everywhere in USA.

If you select 'International Including USA', the plans that show up will cover anywhere outside home country.

e.g., Visitors Care (Fixed coverage) and Protection America (Comprehensive Coverage) plans would cover anywhere outside home country.
