• JADAV (last name) Smita (first name) Dipesh (middle name) is the married name of the beneficiary, after her marriage to JADAV, Dipesh Morarilal. 
  
  
• Copy of Smita’s marriage certificate is enclosed herewith. 
  
  
• Copies of passports 
  
  Includes the earlier expired passport and the most current passport. 
  Shows the names of both parents and the spouse. 
  
  
• Affidavit by JADAV, Smita (beneficiary) indicating that her names mentioned in various documents (before and after her marriage) are for one and the same person. 
  
  Variation in her names before the marriage is due to local cultural customs and not due to any legal name change.
  
  
• Copies of school records that shows Smita’s name before marriage, date of birth, and father’s name
  • Certificate from the school that indicates that she is the bona fide student of the school. 
    
    
  • Copy of the grade 3 mark sheet in local language (Gujarati) and its original translation in English. 
    
    
  • Copy of school leaving certificate in local language (Gujarati) and its original translation in English. 
    
    
  • Distinction certificate for Grade 10th examination of Gujarat government board. 
    
    
  • Medical degree certificate 
    
    School records show name of only the father because it is customary in India to include only father’s name. 
    
    
• Affidavits by close relatives about the birth of Smita.
