All nonimmigrant visa (V visa, K visa etc.) applicants need to fill DS-156, nonimmigrant visa form. Each traveler must have a separate application form and must sign it by hand.

You can either fill the form online and take a print out that contains bar code or you can just fill the paper form on the web site and take a print out or you can print it out and write it by hand.

• Online form with barcode: Once you fill it online, take a print out. It prints the form with barcode which makes it faster for US consulate to process your form.
  
  
• Fillable PDF form: It is a fillable PDF file. You can enter the details online and take a print out or you can first print it and write it by hand.

DS-156 MUST be printed back-to-back on 8 1/2 x 11 paper (standard paper size in US) or on A4 paper (standard paper size in India). Two-paged applications will not be accepted.
